🩺 GP Letters & Forms – Why Is There a Charge?
Why am I being charged?
Patients often request forms or letters from a GP for purposes such as:
• Work or occupational requirements
• Travel or fitness to fly
• Insurance claims
• DVLA or benefit applications (e.g. PIP)
• School, housing or social requests
These services are not funded by the NHS contract.
Because of this, GP practices may charge a fee to cover the clinical time, administration, and responsibility involved in completing these requests.
The fee also reflects the medical opinion and professional responsibility for ensuring all information provided is accurate and appropriate.
Isn’t it just a quick signature?
Unfortunately, no.
Even simple requests require the GP to:
• Review your full medical record
• Check electronic and historical notes
• Ensure all relevant information is correct
• Confirm it is safe to provide the requested statement
For example, a short letter confirming you are fit to travel still requires a full medical review before it can be issued.
GPs are professionally responsible for the information they provide, so this work must be completed carefully.

All medical reports, forms and letters cost £50
📩 What happens after I submit a request?
1️⃣ We receive your request
2️⃣ You will receive a message asking you to confirm that you are happy to pay the fee
3️⃣ Work will not begin until payment is confirmed
💳 Payment
Payment is required when collecting your completed form or letter.
Accepted payment methods:
• Bank transfer
• Debit card
• Credit card
• Cash
❌ Cheques are not accepted
🚫 We are unable to provide:
• HGV, PSV, lorry or taxi medicals/forms
• Power of Attorney documentation
• Passport signing
